Re: Cover Sheets
I usually put the title in the middle, possibly with the number of lines also, and then my name and contact details bottom left. The main value of a cover sheet is so the organisers can maintain a link between poem and poet while maintaining anonymity for the judges. I'm not entering competitions much at the moment, but I have noticed organisers seem to be asking for them less. The entry form often serves the same purpose, requiring the names of the poems entered as well as course as the name of the poet.

Re: Cover Sheets
Hi Wazza
Sometimes the comp states what to put on the cover sheet, e.g. "Cover sheet with name, email address and phone number and title of entry". That's what I do ...
Title of Comp
Title of Section if applicable (e.g. humorous, serious, or themed section)
Title of Poem
My Name
My address
My email
My phone
I don't usually put number of lines unless it is stipulated but it's probably a good idea.
You need a cover sheet for each poem. But if they want three copies of the poem you only need one cover sheet for that particular poem.
As Stephen says, some comps don't bother any more, they use the entry form. Some ask for a separate entry form for each poem.
They are all different - just a matter of reading the conditions and then not getting them mixed up when you are putting the paperwork together for more than one!!
Hope this helps.
Cheers
